ラテン語 cancer（カニ・腫瘍）の借用。腫瘍から周囲へ血管が伸びる様子がカニの脚のように見えたためだと言われ、ラテン語の段階で「カニ」と「悪性腫瘍」の両方を指していた。
英語では医学の「がん」が中心の意味。星座の蟹座 Cancer、北回帰線 the Tropic of Cancer にはカニの意味が残る。比喩では「社会をむしばむもの」（a cancer on society）。
