off-（〜から離れて）+ hand（手）。古英語の ofhende（手を離れた・失われた） = of-（離れて）+ hand（手）+ -e（「〜を持つ」の形容詞を作る） と同じ組み立てで、「手元にない」が出発点。
手元に準備や材料がない状態から、「その場の思いつきで・準備なしに」になった。I can't tell you offhand.（すぐには言えません）のように、副詞で「即答としては」の意味で使うのが最も多い。
形容詞には二つの顔がある。offhand remark（思いつきの発言）と offhand manner（そっけない態度）。準備がない→気楽→無造作→ぶっきらぼう、と少しずつ失礼な方へ寄っていく点に注意。
