古英語 þuruh（〜を通って）から。実は through（〜を通って）とまったく同じ語で、強く発音された形が thorough として分かれた。「端から端まで通り抜けた」→「隅々まで及んだ→徹底的な」。
語根 *terh₂-（通り抜ける）はラテン語側で trans-（越えて）・terminal（通り抜けた先の終端）にもなっている。thoroughfare（通り抜けの道→大通り）・throughout（全体を通して）が親戚で、「トンネルを最後まで通り抜ける」絵で「徹底的」が覚えられる。
