tor はラテン語 torqueō（ねじる・回す）。それを ment が「その働きの結果・その道具」としてまとめ、もとは綱をねじって苦痛を与える拷問具を指した。そこから torment は「激しい苦痛・苦悩」、動詞で「苦しめる」を表す。
体や心が締め付けられて安らげない感覚が核である。mental torment は「精神的な苦悩」、tormented by guilt なら「罪悪感に苦しめられる」。虫にまとわりつかれて悩まされる場面にも使い、軽い気がかりではなく、何度も意識に戻る苦しさを表す。
